St. Bernards Healthcare and Medical Group are excited to partner with St. Bernards Sports Medicine to offer comprehensive student athlete pre-participation evaluations on May 15, 2021 at the First National Bank Arena. These are the multi-specialty evaluations we typically provide at the Health & Fitness Expo.
If you are a student at BAY, BROOKLAND, JONESBORO, NETTLETON, RIVERSIDE, VALLEY VIEW or WESTSIDE, click here to complete the necessary paperwork to register online. SportsWare registration must be complete by MAY 10.
All COVID-19 precautionary measures will be taken. Everyone who enters will be screened and a mask is required. Thank you for understanding.
Athletes and their parents MUST ENTER AT THE YELLOW ENTRANCE. Only clear bags will be allowed in the Arena. You will then proceed to the registration tables.
Physicals are for all student athletes entering grades 7 - 12 during the 2021-2022 school year. This includes current 6th graders planning to play school sports next season.
Wear appropriate clothing - wind pants or gym shorts. No skirts or tight blue jeans. Sports bra recommended for females under their shirts.
