Total Life Healthcare
Total Life Healthcare is a managed care program providing a comprehensive set of healthcare services to individuals 55 and older who can safely reside at home but need long-term care services to do so. The program utilizes the PACE (Program of All-Inclusive Care for the Elderly) model to ensure care and services are coordinated to support program participants, family members, and other caregivers. PACE is a fully integrated, comprehensive model of care that allows older adults who qualify for a nursing home level of care to continue living in the community.
PACE Participants may be financially responsible for care and services not authorized by the PACE Program.
St. Bernards Total Life healthcare is an all-inclusive elderly care program that includes these services and many others:
- Transportation
- Home Health
- Geriatrician, primary care, and specialty physicians
- Hospital care
- Adult daycare
- Pharmacy
- Physical therapy
- Speech Therapy
- Meal delivery
- Lifeline personal help monitor
- Social work
Total Life Healthcare is provided at no cost for participants with Medicaid or Medicare with Medicaid. The program is available for residents living in specific ZIP codes within a seven-county service area.
