To obtain medical records, patients can fill out a Release of Information Form / Formulario de divulgación de información to mail or patients may visit the Medical Center tower located at 225 E. Washington Ave in Jonesboro, AR. Patients are asked to bring a picture ID and check-in at the main registration desk at the St. Bernards Medical Center tower entrance.
