How to Transfer a Patient to St. Bernards Healthcare System

For healthcare providers and hospitals only. Patients must work through their provider or hospital to discuss transfer options.

At St. Bernards Healthcare, we’re committed to making the patient transfer process smooth and efficient. Transfers may be necessary when a patient requires a higher level of care, specialized services, or if there are capacity constraints at your facility.
The center operates 24 hours, seven days a week to support providers with seamless patient transfers.

St. Bernards offers direct admissions and emergency-department-to-emergency-department transfers for all adult and pediatric patients within our healthcare system.

What to Expect:

- Intake: Our nurse gathers essential patient details (e.g., name, dob, diagnosis) from the sending provider.
- Provider Connection: We connect the sending provider with the receiving provider to discuss acceptance and any capacity constraints.
- Bed Coordination: We work with the receiving hospital to secure an available bed.

Please note: Some service referrals, such as psychiatry, may require additional information.
We aim to respond quickly to your transfer request, though various factors may affect priority, placement, or cause delays.
