Our experienced team provides expert care for aging minds and bodies—delivered with respect and compassion and focused on maintaining the highest quality of life possible. The Senior Health Clinic team includes a geriatrician, an advanced practice nurse and a social worker.
Together, they provide consultations, health assessments and treatment for senior adults, based on their individual needs and circumstances.
Geriatricians are specially trained to manage multiple serious medical problems. Additionally, geriatricians are trained to identify medications which may not be appropriate for older adults. Geriatricians lead a team that help patients and families navigate the experiences and needs that growing older presents.
Because older patients have multiple medical problems, appointments times are longer so that all needs can be addressed. Staff are familiar with resources that specifically target the older population. The patient and family are very much a part of the care team for patients at the St. Bernards Senior Health Clinic.
