Requests submitted will be processed by our clinic staff as soon as possible. You will hear from us within one business day.

This should not be used in the case of an emergency. If you are having an emergency, please call the clinic or emergency personnel (911) directly.

You can request refills of your prescriptions by following the directions below: 

1. Call (870) 932-1198 or toll free (888) 798-0700.
2. Be prepared to leave the following information: Caller’s Name, Callback number, Patient’s Name, Patient’s Date of Birth, Medications to be refilled, and the Pharmacy you would like to use.
