Qualifying Criteria:
- Age 50-77
- Tobacco or smoking history of at least 20 pack-years*
- Current smoker or quit smoking within the last 15 years
- Asymptomatic (no signs or symptoms of lung cancer)
To calculate pack-year: ________ years smoked x _________ packs per day
