I remember…

When I took a metal cart to every floor to pick up discharged patient charts that were as thick as large encyclopedias! I would take them to Medical Records where they’d be put in order by hand, then every abbreviation on the face sheet written out and the chart would have someone hand tag every signature line for the doctors. The charts were then put in cubbies for them to sign.
