Purpose and Availability

• The purpose of this program is to prevent extended, chronic or unnecessary hospitalization to patients by providing medical support and rehabilitation in the home setting under the direction of the physician, when the environment is determined to be conducive to the patient’s total rehabilitation.
  
  
• Available to persons in Craighead, Greene, Lawrence, Poinsett, Randolph, Mississippi, Jackson and Cross Counties.
