In support of our mission to provide Christ-like healing to the community through education, treatment and health services, St. Bernard's offers a comprehensive diabetes education program which is recognized by the American Diabetes Association (ADA) for meeting national quality standards for diabetes education. No other program in this area carries this recognition. To obtain recognition, diabetes programs must meet nationally set standards regarding curriculum and a staff of knowledgeable health professionals.
Diabetes is a disease that is very much controlled by the patient through diet and other lifestyle choices. Therefore, self-management education is an essential component of diabetes treatment. The participant in an ADA recognized program will be taught self- care skills that will promote better management of his or her diabetes treatment regimen, based on individual needs. All approved education programs cover the following topics as needed: diabetes disease process; nutritional management; physical activity; medications; monitoring; preventing; detecting and treating chronic complications through risk reduction; goal setting and problem solving; psychological adjustment; preconception care and management during pregnancy, and gestational diabetes management.
Staff:
Staffing includes multidisciplinary resources including an Advanced
Practice Nurse, RN Educator, Dietitian, Pharmacist, and Exercise
Physiologist.
Classes:
Classes are offered by physician referral monthly in both individual and group settings, with the complete program consisting of seven hours of education. We ask that patients bring their glucometer, glucose log and medications with them to the appointment. Family members are welcome and encouraged to attend.
Cost:
Insurance coverage varies, but most commercial plans cover at least some of the cost.
Medicare part B does cover the cost of the program for qualifying participants with referral from their physician.
Location:
Classes are held in the diabetes education office at 505 E. Matthews, Ste 200.
We also hold classes at the Clopton Clinic, 300 Carson Street for patients of that clinic.
Additional Support:
As an additional service, we have a diabetes support group that is free of charge and open to the public on the second Tuesday night of the month in Stroud Hall, which is located in the annex building. Various speakers on diabetes topics are featured at these meetings.
For more information, you may call the diabetes education office at 870-972-4341.
